What a Below-Knee Prosthesis Is Made Of
A transtibial limb is five parts working together, and a weakness in any one of them is felt in all the others. Understanding what they do makes the fitting conversation much easier to follow.

Components can be changed later without rebuilding everything. A foot can be upgraded as activity increases; a socket usually cannot, because it is made to the shape of your limb at the time of casting.
Socket Designs and Suspension Options
Socket designs
Two broad approaches are in common use. Patellar tendon bearing (PTB) sockets load specific tolerant areas — principally the patellar tendon — and relieve the bony and sensitive ones. Total surface bearing (TSB) sockets spread load across the whole limb surface instead, usually with a silicone liner.
Neither is better in the abstract. The choice depends on limb shape, soft-tissue volume, skin condition, sensation and how long you will be on your feet. That decision is made at assessment, not from a web page.
Suspension
- Pin and shuttle lock — a pin on the liner clicks into a lock in the socket. Secure and easy to judge; needs enough limb length.
- Suction — a one-way valve expels air to hold the limb in place. Close coupling, but needs stable limb volume.
- Suspension sleeve — a sleeve rolled over socket and thigh. Simple and forgiving; warmer to wear.
- Supracondylar cuff — a strap above the knee. Straightforward and tolerant of volume change.
If your limb volume changes through the day, that is normal and is managed with stump socks of different ply rather than by forcing the socket.
From Amputation to Walking: What the Stages Look Like
People ask how soon they can be fitted more often than any other question. The honest answer is that it depends on healing, and on how quickly limb volume settles — not on a fixed calendar.
- Healing. Nothing is cast until the wound is closed and your surgical team is satisfied.
- Shaping and shrinkage. The limb loses volume as swelling settles. Casting too early produces a socket that is loose within weeks.
- Casting and fitting. The limb is cast or scanned, a test socket is made, and the fit is adjusted before anything is finalised.

- Review. The first socket is rarely the last. Limbs keep changing, especially in the first year.
